SERPA-ACO will be a leader in improving the health of our communities through the delivery of high quality, team-based and coordinated care. Patients will receive high quality care in their own communities that meet their needs at a lower total cost. We will help clinics build sustainability for clinic‐based care coordination and systematic quality improvement efforts.
In 2012, SERPA-ACO was established to do the Center for Medicare and Medicaid Services: Medicare Shared Savings Program. We began conducting value-based care programs in 2013 working with both government and commercial payers.
